FBI Captures Fugitive Accused in $3.7 Billion Medicare Fraud Scheme

MIAMI — The FBI has apprehended a major fugitive wanted in one of the largest Medicare fraud cases in U.S. history.
Ibrahim Khaldoon Hilmi is accused of orchestrating a massive scam that allegedly defrauded taxpayers of $3.7 billion. He had been on the run since May 2025 before being tracked down overseas and returned to the United States through a Foreign Transfer of Custody.
FBI agents were seen escorting the suspect, dressed in blue medical scrubs, from a private jet upon his arrival on U.S. soil.
FBI Director Kash Patel commented on the arrest, stating:
“Any criminal actor who steals from the American taxpayer will be caught, no matter where they try to hide.”
The operation involved the FBI’s Miami office, the Justice Department, Turkish authorities, and the White House fraud task force led by Vice President Vance.
Hilmi now faces charges in the United States for his alleged role in the expansive fraud scheme. The case underscores ongoing federal efforts to combat large-scale healthcare fraud and hold accountable those who exploit government programs.
