Derek Pfaff’s Second Chance: Face Transplant Restores Life After a Decade of Trauma

In March 2014, 19-year-old Derek Pfaff, a college student from Harbor Beach, Michigan, suffered a self-inflicted gunshot wound that nearly ended his life and left his face catastrophically damaged. The injury destroyed much of his facial structure, including his nose, upper and lower jaws, teeth, and significant soft tissue. Over the following decade he endured 58 reconstructive surgeries, yet essential functions remained lost: he could not smell, blink normally, speak clearly, or eat solid food, relying instead on a feeding tube.
Everything changed in February 2024. A multidisciplinary team of more than 80 specialists at Mayo Clinic in Rochester, Minnesota, performed a groundbreaking face transplant lasting more than 50 hours. Led by facial reconstructive surgeon Dr. Samir Mardini, the operation replaced approximately 85 percent of Pfaff’s face with donor tissue. The transplant included parts of the forehead, upper and lower eyelids, nose, cheekbones, upper and lower jaws with teeth, facial muscles, nerves, and skin of the face and neck. Advanced planning tools—digital surgical models, 3D-printed cutting guides, and detailed nerve mapping—helped achieve precise alignment and functional restoration.
The results have been transformative. For the first time in ten years, Pfaff can breathe through his nose, blink, swallow, smile, and eat solid food. Sensation and movement continue to improve as nerves regenerate. He has described the outcome as a “second chance” and a return to “normal life.” Exactly ten years after the original injury, on March 5, 2024, he was finally allowed to see his new face.
Face transplants remain extremely rare—fewer than 60 have been performed worldwide. Pfaff’s procedure was only the second of its kind at Mayo Clinic. Beyond the technical achievement, the case highlights the profound impact of organ and tissue donation. Pfaff has publicly expressed deep gratitude to his anonymous donor and the donor’s family.
Today, the 30-year-old uses his experience to advocate for mental-health awareness, resilience, and the importance of organ donation. He speaks openly about the pressures that led to his suicide attempt and encourages others facing similar struggles to seek help. His story demonstrates both the limits of conventional reconstruction and the life-restoring potential of facial transplantation when conventional options fall short.
